Hellooo Freshies!
Just a gentle reminder that you need to give your names and money to your respective in-charges by this Friday (5 June) for NRA's day. take note that once you have given your names, you are required to pay. There will be no refunds for any last minute backouts ^.^
You are also required to collect 3 forms from your respective In-charges:
1. Indemnity Form
2. Permit to stay overnight Form
3. What to bring Form
Please fill up forms 1 & 2. Return them to your respective in-charges by next Wednesday (10 June) latest 8pm.
Next, fill up the following blanks as follows on your forms:
1. Indemnity Form, in the first section:
- Organising student club/ society: New Revolving Age Hip Hop Dance
- Title of activity: NRA 9th anniversary camp
- Nature of activity: Overnight Stay (please circle)
- Start date / Time of activity: 19th June 2009 / 9am
- End date / Time of activity: 20th June 2009 / 2pm
- Venue of activity: Ngee Ann Polytechnic
- Country / City of activity: Singapore
2. Permit to stay overnight Form:
- Organising student club/ society: New Revolving Age Hip Hop Dance
- Title of activity: NRA 9th anniversary camp
- Nature of activity: Overnight Stay (please circle)
- Start date / Time of activity: 19th June 2009 / 9am
